For patients under the care of Affilia Home Health, we ask that they take on certain responsibilities in their care and treatment. This goes a long way in ensuring you receive timely treatment and the best support from our team of nurses and therapists.

For patients under the care of Affilia Home Health, we ask that they take on certain responsibilities in their care and treatment. This goes a long way in ensuring you receive timely treatment and the best support from our team of nurses and therapists.

Review each of the following safety tips and note the ones you need to work on:

Provide complete and accurate information to the best of your knowledge about your present complaints and past illness(es), hospitalizations, pain, medicines, allergies, and other matters relating to your health.

Supply medicine, equipment, or supplies that we are unable to provide.

Remain under a physician’s care while receiving home health services.

Notify us of changes in your condition. This includes unexpected changes, like a hospitalization, changes in the plan of care, symptoms to be reported, pain, homebound status, or change of physician.

Follow the plan of care and instructions and accept responsibility for the outcomes if you do not follow the care, treatment, or service plan.

Cooperate with all agency personnel and ask questions when you do not understand about your care, treatment and service, or other instruction about what you are expected to do. If you have concerns about your care or cannot comply with the plan, let us know.

Report pain. Be sure to discuss pain, pain relief options, and your questions, worries, and concerns about pain medicine with our staff or appropriate medical personnel.

Notify us of schedule changes. Be at home when our staff arrives or to tell us if your visit schedule needs to be changed due to a medical appointment, family emergencies, etc.

Tell us of other services – to tell us if you are receiving services from another home care or outpatient service provider.

Notify us of insurance changes – to tell us if your Medicare or other insurance coverage changes or if you decide to enroll in a Medicare, a private Health Maintenance Organization (HMO), or hospice. Failure to notify Affilia of changes in insurance coverage may result in you being financially responsible for services/supplies provided

Promptly meet your financial obligations and responsibilities agreed upon with Affilia. You can easily pay your bill online.

Furnish financial information – to assist us in determining a fair fee if you are not covered by any health insurance and if you feel you cannot pay for services.

Inform us of changes in advance directives – to inform us of the existence of, and any changes made to advance directives.

Advise us of problems or dissatisfaction with the services provided. Call your local office and ask to speak to a supervisor.

Provide a safe environment – to provide a safe and cooperative environment for care to be provided. This includes confining pets, securing weapons, and not smoking while our caregivers are present. It is expected that you will not take drugs that have not been prescribed by your physician and that you will not complicate or endanger the healing process by consuming toxic substances during your admission. We love pets, too, however, we will ask you to place your pet in another area while we are in your home. This is both for your safety and that of our staff. Pets can increase your risk for falling during therapy, add the potential for infection during wound or IV care, and cause distraction during our visits.
