Telehealth technology allows us to monitor your health conditions when our staff is not present. If your home healthcare plan indicates the use of telehealth, we will set up a monitor in your home to track vital signs such as blood pressure, heart rate, and weight.
This monitor not only alerts you when it is time to check these vital signs, but also is programmed to ask questions about your specific health condition. A nurse in our office reviews this information and calls you about any notable changes. These changes may indicate a need to have one of our nurses contact your doctor or visit you.
